Indirect coomb test
The indirect Coombs response (indirect Coombs) detects aberrant circulating antibodies in the patient’s serum that may react against transfused red blood cells. Antibodies directed against blood types A, B, and O are different. The indirect Coombs response differs from the direct Coombs reaction in that it recognizes antibodies that have already been bound to red blood cells. The patient’s serum is considered the source of the antibodies in this test, and the donor red blood cells are the cells-antigens. The serum and red blood cells are mixed together to allow any antibodies to adhere to the red blood cells before adding anti-human globulin. Adhesion will occur if the patient’s serum contains antibodies that react with and bind to the donor’s red blood cells, and the test will be considered positive. If there is no adhesion, there is no antigen-antibody response. Although the serum contains antibodies, the donor red blood cells lack the antigen to which the antibodies will react.
