During the pregnancy, there might be a blood group incompatibility between the mother and the fetus, this usually happens when the mother has Rh-negative and the father has Rh-positive. If the fetus inherits the Rh antigen enters from the father to the mother’s bloodstream during pregnancy or childbirth, the mother’s immune system may produce antibodies against the Rh antigen. The indirect Coombs test helps detect the presence of these antibodies in the mother’s blood.

How is an Indirect Coombs test done?

  • Sample collection: A sample is collected from the patient, usually from the vein in the arm.
  • Separation of the serum: The collected blood sample is centrifuged to separate the serum, which is the yellowish liquid portion of the blood, from the red blood cells.
  • Mixing the patient’s serum:  A small amount of serum is added to each test tube that contains red blood cells. The mixture allows antibodies present in the patient’s serum to react with the antigens on the red blood cells.
  • Incubation: The test tubes are incubated at a specific temperature to allow sufficient time for the antibody-antigen reactions to occur.
  • Addition of Coombs Reagent: This Coomb Reagent is added to each test tube, The Coombs reagent consists of antibodies that can bind to antibodies present on the patient’s red blood cells, forming visible clumps.
  • Observation and interpretation: The test tubes are observed for agglutination.  Agglutination indicates a positive result, meaning that the patient’s serum contains antibodies that have reacted with the red blood cells in the test tube. A negative result indicates the absence of detectable antibodies in the patient’s serum.
